You can use this form to submit an Application for Membership in the ODHFS. Once you fill out the form you can either submit your dues payment online or you can send a check with your membership information to the ODHFS. To submit your dues payment by PayPal or Credit card ($16) please click here: https://www.odhfs.org/store.html (PLEASE NOTE: Dues payments submitted online are charged a $1 processing fee to cover our PayPal costs). To pay your dues by check, please mail the check ($15) with your contact information and note that you submitted an application online to: ODHFS, P.O.
